You can unlock the Wrack and Ruin quest in Diablo 4 by finishing the Tainted Flesh quest and speaking with Timue. Your next task is to find the temple, which is located near the town of Yngovani. Once inside the town, you will find the temple to the east. Once you have arrived, you will be tasked with finding the serpent statue’s, guarding the temple’s entrance, left and right eyes. While finding the Left Eye is frustrating, the quest will mark the location on your map with a gold circle. This will at least give you a general area of where to look for both eyes.

Where to find the serpent’s left eye in Diablo 4
Once there, you must loot the Half-Devoured Cultist sitting on the steps of the Destroyed Serpent Shrine to find the Serpent’s Right Eye.

To find the left eye in Diablo 4, you must talk to the Dying Bandit at the Thieves’ Encampment, north of the Snake shrine.
