At the time of writing this guide, there are at least 3 possible ways to farm them. Some aren’t as fast but have a higher drop rate. They are:

  • Slaying corrupted enemies
  • Outgrowths in Milignants Tunnels
  • Investing in the Urn of Milgance

Slaying corrupted enemies: Partially corrupted enemies may drop the hearts after you beat them. While they don’t do it constantly, there’s a large number of them so the more you take down, the higher a chance they’ll drop a Malignant Heart. However, if you collect them with the Cage of Binding ritual the creatures will respawn as Fully Corrupted. When they’re in this form, they’ll also drop the hearts. So you could get them quickly by resetting the area with this method.

Outgrowths in Maligant Tunnels: As part of the first season, there are tons of side things you can do. One of those things is these Malignant Tunnels which are deep tunnels that go under Sanctuary. When you’re fighting monsters in these tunnels, you’ll come across 2 Outgrowths in every borrow. Using Malignant Invoker that you can craft at Cormod’s Wagon, you can interact with these Outgrowths and they’ll spawn fully corrupted enemies. Upon slaying them, they’ll drop Maligant Hearts.

Investing in the Urn Maligance: By doing this, the Miligant Hearts will drop at a higher rate. While this may take more effort over time, it’s worth it when you do.
