Play the hero when it’s necessary

While sometimes Karlach will do what’s necessary, even if it may seem barbaric, she’s a sweet-hearted hero. If you come across NPCs that ask for your aid, and they seem in dire trouble, she’ll probably approve of you helping. Do good deeds, and Karlach will be happy.

Especially when it comes to children in need, and especially when it comes to hurting cruel people. Her number one enemy is Gortash, after all. Anytime you come across a bully who’s cruel for no reason, she’ll approve of taking them down. Violence is sometimes the right answer, but only in specific situations.

Be kind and help the Tieflings

As a fellow Tiefling, Karlach will be extremely opposed to helping the Goblins in any way. She’ll also want to help the Tieflings, so if she’s in your party and she spots you being unkind or cruel to them, she won’t take that lightly. This typically goes for any Tiefling you come across. Don’t worry about her opinion on helping the Grove, you can still help the Druids and get her approval, as it’s the heroic thing to do.

Don’t side with Gortash

Karlach has a bad past with Gortash, as he sentenced her to a life of hell in, well, hell. Finally, she escaped from Avernus, and if you ever dare to make peace with Gortash, she’ll feel betrayed. At some point during Act 3, you can make a deal with Gortash. Saying yes to this deal will result in Karlach’s disapproval, so be careful who you side with.

Help her deal with her Infernal Engine

She can’t survive forever on the material plane, but she can find a temporary solution. If you help Karlach through her companion questline, and find Infernal Iron for her, she’ll be very pleased. And in her darkest moments, when she feels all hope is lost, pick her back up. Be a friend to her, or maybe even a romantic partner.
