Surprisingly, the Maid Dress is challenging to know how to unlock and even harder to actually unlock. You get the Maid Outfit by cleaning up poop and throw up in Cult of the Lamb. On Reddit, I found that people needed to clean around 100 times to get the Maid Dress, so keep up the good work and you’ll get it eventually.

Clean up poop and throw up
To make this go quicker, I highly recommend getting rid of all your bathrooms and janitor stations so you can clean up the poop and throw up yourself. This will increase your chances of unlocking the Maid Outfit quickly. Also, the higher level your broom is, the better your chances.
All the unlockable outfits are acquired through doing the thing that the outfit is tied to. That’s why it makes sense that the best way to get the Maid Dress in Cult of the Lamb is by cleaning up your Followers’ waste.
Buy it from Berith
The only other way to unlock the Maid Dress in Cult of the Lamb is by buying it through Berith, the caterpillar vendor that can appear in any dungeon. This method relies much more on luck because there is no guarantee you’ll find Berith or that they’ll sell the Maid Outfit, but it’s worth mentioning.
How to craft the Maid Dress in Cult of the Lamb
Once the Maid Dress is unlocked, you need to craft it with six Silk Thread, six Cotton, and one Sin. You create Silk Thread from Spider Silk at a Refinery, you get Cotton from Cotton Seeds on a farm plot, and there are many ways to farm Sin, but the easiest is through Rituals.
Every time you want to make a Maid Dress, you need to pay those resources, so it does get pricey. To help you get a lot of Sin quickly, make sure you have the best Sin Doctrines available.
