Although these hearts won’t play a factor into your build, they have other purposes. Let’s go over what they do and how to find them.
Diablo 4: How to find and use Malignant Hearts in Season 2
If you want to face off against an Endgame Boss, you first need to summon them. These are huge World Bosses that require you to be at World Tier 3 or above. How you can get Malignant Hearts efficiently in Diablo 4 Season 2 is by completing quests for the Tree of Whispers or defeating Grotesque Debtors.
Once you finish the Diablo 4 campaign, you gain access to Whispers of the Dead. These are smaller quests that’ll reward you with Grim Favors, and if you get 10, you can earn yourself a Whisper Cache from the Tree of Whispers. You have the chance of earning a random Malignant Body Part:
- Blackened Femur
- Gurgling Head
- Malignant Heart
- Trembling Hand
Unfortunately, the only way to get the Malignant Heart is to be on World Tier 4. But who will you summon with this heart? You can actually summon the Echo of Varshan during World Tier 4. Upon defeating this Endgame Boss, you have the chance to earn Unique items.

Another way to get a random Malignant Body Part is by killing Grotesque Debtors. You can find these enemies in Blood Harvest events, as well as areas where you can earn Grim Favors.
Once you have yourself a Malignant Heart, head inside the Malignant Burrow. This is a location inside The Writhing Mire, which is right beside the Tree of Whispers. Not only are Malignant Hearts used for summoning the Echo of Varshan, but you can use them as crafting materials. Through an Alchemist, you can turn it into any other Malignant Body Part.
